Every semester, H.O.M.E. welcomes new faces at Pat Crowley and Nathalie Salmon Houses.

H.O.M.E. partners with Loyola University Chicago's Marcella Niehoff School of Nursing to provide a unique experience for students and our residents. Through the Loyola Community Nursing Center, students are paired with nursing faculty and provide services to the community, including home visits to seniors who live in our shared living residences. Their services include a comprehensive assessment of health and social needs, and linking clients with community resources.

The students work closely with seniors, their families, and our housing coordinators to provide seniors with needed resources and information that enables our clients to make informed decisions about their health. Meanwhile the students learn the ins and outs of caring for patients in the community as well as gain experience working with the older adult population.

The students shared a recent experience at the Pat Crowley House:

"On Tuesday, November 4, 2014, the nursing students from the Loyola University Chicago shared a presentation with the residents at the Pat Crowley House. The nursing students make weekly patient visits, but this particular visit included discussion on how to prepare for your doctor’s visit. The presentation consisted of topics such as creating a list of questions for your doctor, writing down current medications, and bringing someone to your visit in order to help with questions or writing down information. Each resident received a “check list” designed by the nursing students which included the discussed topics. Click here for the full check list.

The residents shared positive feedback, noting that the information was helpful because they frequently visit their doctor's offices and have a hard time remembering their questions. In fact, some residents mentioned how they've already been practicing some of these techniques. Afterwards, residents participated in a good discussion amongst themselves sharing their own ideas of what is beneficial during their visits.

The nursing students were excited to see the residents conversing with each other regarding their experiences. They hope the residents will have more productive doctors' visits which will improve their overall health."

H.O.M.E.'s partnership with the Loyola Community Nursing Center is a wonderful resource for our seniors. We work with several community partners to offer all kinds of resources to the older adults who we work with:

University of Chicago, SSA

Chicago Cares

University of Illinois-Chicago, Medical School

Oak Street Health

Glenwood Sunday Market

And many others!
